CARE SERVICES

Care Services are provided at no cost to support the best quality of life for those infected and affected by HIV/AIDS in Berks and Schuylkill Counties. Services, available in English and Spanish, are comprehensive and include the following:

CASE MANAGEMENT -- assessing needs and integrating clients into the system:

• introduction to CCWS services
• individual needs assessment & service planning
• crisis intervention
• advocacy as client navigates the "system"
• supportive housing program
• service referrals to other area agencies
• exploring long-term options for improving quality of life

WELLNESS SERVICES:

• medical case management for continuum of care
• patient health education
• medication education
• support groups for people infected or affected by HIV/AIDS

DIRECT ASSISTANCE

• a drop-in center for community building and connecting
• a food pantry and nutritional supplements
• financial assistance to help with emergency needs (housing, medications, eyeglasses)

DROP-IN CENTER (at BAN Only)

• trained clients with HIV/AIDS act as peers in the drop-in center each day to assist and empower other clients
• a place to connect with other and build a community
