CARE SERVICES
Care Services are provided at no cost to
support the best quality of life for those infected and affected by HIV/AIDS in
Berks and Schuylkill Counties. Services, available in English and Spanish, are
comprehensive and include the following:
CASE MANAGEMENT -- assessing needs and
integrating clients into the system:
- introduction to CCWS services
- individual needs assessment & service
planning
- crisis intervention
- advocacy as client navigates the
"system"
- supportive housing program
- service referrals to other area
agencies
- exploring long-term options for
improving quality of life
WELLNESS SERVICES:
- medical case management for continuum
of care
- patient health education
- medication education
- support groups for people infected or
affected by HIV/AIDS
DIRECT ASSISTANCE
- a drop-in center for community
building and connecting
- a food pantry and nutritional
supplements
- financial assistance to help with
emergency needs (housing, medications, eyeglasses)
DROP-IN CENTER (at BAN Only)
- trained clients with HIV/AIDS act as
peers in the drop-in center each day to assist and empower other clients
- a place to connect with other and
build a community
